Dr Simon Forman (1552-1611) the celebrated 17th century magician, astrologer and physician lived in Lambeth from 1601 until his death, which he predicted. He was said to be 'very judicious and fortunate in horary questions and sickness'. Forman was known to practise medicine by astrology, sometimes making effigies of his patients and foretelling their diseases. He was often accused of being a 'quack' and fined or imprisoned.Engraving after the original painting by Godfrey, published July 1st 1776.
